WINTER SPORTS ANNOUNCEMENT!

***PLEASE NOTE- all dates are tentative and subject to change with directives from Governor Murphy. We will update the website as we continue to receive guidance from the NJSIAA***

ALL BURNET MIDDLE SCHOOL STUDENT-ATHLETES SHOULD RETURN THEIR SPORTS PHYSICAL TO BMS:

ALL students MUST complete the newly revised Health History Update Questionnaire that contains questions regarding COVID-19.
Sports physicals are only valid for 365 days from the date of the last sports physical.

If you have had a sports participation physical during the 2019-2020 school year and it does not expire BEFORE, THE BEGINNING OF YOUR DESIGNATED SEASON, you MUST complete the following:
- Health History Update Questionnaire prior to the start of the season.
If the student-athlete has NEVER submitted OR has a Sports Physical that expires before December 3, 2020, they must submit a NEW Sports Physical PRIOR to the start of the WINTER SEASON this includes:
- Pre-Participation Examination Form AND
