- 4 other Items with Name & Address (Current Utility Bills, Homeowner's
Policy, Pay Stub w/Name and Address, Bank Statement, Car Insurance
Policy, Credit Card Bill, etc.)

Proof of custody forms for Guardian/Custodial Parent

For Pre-K Registration - Child must be be of
age by Oct. 1st
Cost: $300.00 for Union residents, $5,295.00 for non-residents
(CASH, MONEY ORDER OR CERTIFIED CHECK ONLY)

Busing / Transportation Information |
|
Transportation shall be
provided for all children in K thru 5 if they live over 1.5 miles from
the school. For children in grades 6 thru 8 you must reside over 2
miles and in grades 9 thru 12 you must reside over 2.5 miles. Any
further questions, please call transportation at 908-851-6447. |
