Fall Sports Registration How To:

ALL students MUST complete the newly revised Health History Update Questionnaire.
Sports physicals are only valid for 365 days from the date of the last sports physical.
If you have had a sports participation physical during the 2021-2022 school year and it does not expire BEFORE, THE BEGINNING OF YOUR DESIGNATED SEASON, you MUST complete the following:
- Health History Update Questionnaire prior to the start of the season.
If the student-athlete has NEVER submitted OR has a Sports Physical that expires before August 10, 2022 they must submit a NEW Sports Physical PRIOR to the start of the FALL SEASON this includes:
- Pre-Participation Examination Form AND
