REGISTRATION INFORMATION

• Parents/Guardians must provide a photo ID to complete the registration process
•  Mandatory Proof of Residency is required

 Must complete the online registration process in its entirety with the necessary documentation uploaded - Registration will be considered incomplete without the upload of documentation

• Original Birth Certificate
•  Medical Records and Immunization Records
•  Proof of Custody, if applicable
•  Legal Custody Documentation, in the event of a divorce or separation
• Transfer Card from prior school and MUST include NJ SMART ID#
•  Last Report CardCopy of current standardized test scores

ALL 5-C and/or 6-C Affidavits MUST be brought to the Residency Office for approval!
